Health Care Directives
An Advance Health Care Directive is a legal document which outlines an individual's wishes upon various forms of incapacity. With it, you can dictate what procedures you want taken upon your incapacity, give directions to your healthcare professionals, and designate who you want making decisions for you.
HIPAA Waivers
The H.I.P.A.A., or Health Insurance Portability and Accountability Act, is legislation passed by the United States Congress which seeks to ensure your medical privacy in an increasingly digital world. However, with respect to estate planning and your wishes, this can often complicate matters. This waiver is deisgned to give the right people information about your medical history when they need it, while still protecting you from those you don't want seeing it.
Important Information Regarding Healthcare Documents
Chances are you already have a healthcare directive and HIPAA waiver through our firm as part of your estate plan. It is important to review them at least every three years to ensure that they are up to date and still accurately reflect your wishes. Your documents can be found in your estate planning binder under the “Advance Healthcare Directive(s)” and “HIPAA Waiver(s)” tabs. Alternatively, we would be more than happy to discuss these documents with you in person.

This service provides you with a card to carry in your wallet indicating your healthcare preferences as well as a 24-hour phone number any doctor can call to quickly retrieve your documents.
